This has happened to me couple times now. Don't exactly remember how the first time, but I assume it would be the same as the second time I did it. The second time happened after dying from failing to swing to one of the platforms or somehow glitching out and swinging too low for the final platform and being revived by a feather.

    This bug is 100% replicable, and also works when you use a goddess grace. You fall off the side of abom, mid chain, either by an attack or the platform shifting at an awkward timing, and you revive yourself with a grace or get revived by a feather and you're stuck on the ground. /escape doesn't even help because you're apparently in combat http://puu.sh/wuZ2Q/7e6322ae47.jpg
