Important notes:
• Provided enhancement success chances are approximate and can be incorrect, as they cannot be checked in-game.
• After Rise update, items below level 90 can be enhanced up to +12 only, so higher enhancement for these were made earlier.
• Changing main composition material (weapon or armor essence) to other rarity tier (in example: from superior(purple) to rare (orange)) will reset enhancement level, but not the other improvements.
• 90 or higher level items must be composed of full rare (item name must be orange) material to enhance them to +13 or higher.
In one of upcoming updates we are likely to get more enhancement stones, that will have high success chance for +11, +12 and +13 90 level weapons. They will be available in Seal of Bravery shop. The +13 one has 100% chance, but permanently binds the weapon, as if it was restored, therefore cannot be enhanced further.
As side note, one of recent updates reduced the negative effects caused by Destruction Safeguard Rune on failure.

All anyone who is not a certifiable, pathological gambling addict needs to know is, 1) simply ignore all of this and, 2) enhance up to +8 (...if you're feelin' lucky, punk!) and move on (...to a different game!).
Updated 90+ level weapon and armor enhancement rates. Since these values are not displayed in the game, we cannot be sure if they are valid. However, it's been around year since Rise update release, so from players experience we can tell they are close. Can't tell about shield though.
New content is in Korean test server - season finale, level 100, increased enhancement chance after each failure, some "steel" used to enhance restored items.... and apparently accessory enhancement:
Accessories follow different enhancement rules and don't provide as significant stats.
Since it's test server - this is subject to change, no point going into details.
Since last update, levels 100, 105, 110 were added and soon 115. During this time, enhancement levels from +16 to +20 were discontinued, and stats readjusted. The enhancement feature now shows more information and has fail-stack system, which increases success rate of next attempt after a failure, which a 100% chance after certain amount of failures at given tier.
